Kindergarten Services

We are proud to be able to offer both brief/screener and comprehensive assessments for kindergarten-aged children. See below for the available assessment options. Please reach out to your educators and/or the Kalos Therapy team to discuss further if you are unsure which assessment/s might be suitable for your child.

If you would like to go ahead with booking any kindergarten services, please submit the referral form at the bottom of this page, or click here. There is an option to request further discussion with our OT Emma if you are unsure which service may be beneficial for your child.

 

Please note that the each service includes a letter with the assessment scores, but no interpretation. For a brief interpretation and recommendations, the additional brief summary report should be requested. If required, each service will include a letter for your kindergarten centre to access inclusion supports.

Includes both the ASQ-3 (assessment 1) and the ASQ:SE-2 Social Emotional form (assessment 2)

This is a comprehensive fine motor and gross motor assessment with various subtests - all or only some subtests can be completed.

Domains
Two Assessments Completed:
  1. ASQ-3: Communication, Gross Motor, Fine Motor, Problem Solving, Personal-Social Development
  2. ASQ:SE-2: Social Emotional
Activities of Daily Living (e.g., self-care) and Instrumental Activities of Daily Living
Sensory Systems: Auditory, Visual, Touch, Movement, Body Position, Oral (taste and smell); Behaviours: Conduct, Social Emotional responses, Attention
This assessment would typically only be completed if there are definite concerns regarding the child's motor skills (e.g., concerns picked up on the ASQ).

Subtests include Fine Motor Precision, Fine Motor Integration, Manual Dexterity, Upper Limb Coordination, Bilateral Coordination, Balance, Strength, and Dynamic Movement

  1. Therapist completes assessment 1 activities with child and parent completes assessment 2 form online OR
  2. Parent completes assessment 1 activities with child and fills out online form for therapist to score, and completes assessment 2 form online

Here’s the process:

  1. Consider which of the above services may be beneficial for your child. If you are unsure, discuss with your educators
  2. Complete our referral form here
  3. We will reach out to you to discuss your needs and confirm which assessment/s will be completed
  4. We will send out a service agreement with final services and costs
  5. Once we receive the signed service agreement, appointments will be offered and booked
  6. After the service is completed, you will receive an invoice for payment
  7. Once payment is received, the assessment score letter will be provided, along with the additional summary report (if requested) and a letter for your centre to support access to inclusion supports (if required)
