Occupational Therapy Assessments and Reports
We offer a variety of occupational therapy (OT) assessments and reports including:

Standard OT Assessment
Suitable for all ages. Depending on age may include assessments such as the Vineland-3, PEDI-CAT, BOT-3, Beery VMI, Sensory Profile 2, or SPM-2. The accompanying report will give assessment scores and a description of functioning across learning, self-care, motor, self-management, and social domains, though not as comprehensive as an FCA. This assessment option is suitable when you are wanting a generalised OT assessment to see where you or the person you support are sitting functionally, and to be guided with recommendations on the next steps and future needs.
Cost: Standard OT Assessments typically take 6 – 8 hours depending on complexity and/or if travel is required, totalling $1,163.94 – $1,551.92.
Sensory Assessment
Suitable for all ages. The assessment will include completion of either the Sensory Profile 2 or SPM-2 and will include a detailed analysis of the individual’s experience and response to different sensory stimuli (including tactile, auditory, taste and smell, visual, proprioceptive, and vestibular stimuli). An interoception assessment (the MAIA) may also be completed. The report will also include recommendations to manage any functional challenges that are impacted by the individual’s sensory profile.
Cost: Sensory Assessments typically take 3 hours, totalling $581.97.
Autism Assessment
Our autism assessment and report can be used as evidence to help an autism diagnostician (paediatrician, clinical psychologist, or psychiatrist) to make a formal diagnosis. Our autism assessments include a variety of assessments depending on your age and presentation. All assessments will include the MIGDAS-2, the SRS-2, and a sensory assessment. Some diagnosticians prefer to administer their own autism assessments, so please check with your specialist prior to booking your appointment with Kalos Therapy.

Functional Capacity Assessment (FCA)
Typically best for NDIS participants aged 7 and above. FCAs include a comprehensive assessment and detailed report to outline the individual’s functioning across various daily living domains. It is more comprehensive than a Standard OT Assessment but may include some of the same assessments. FCAs are typically preferred or required for scenarios such as when an individual is first accepted onto the NDIS scheme (to identify which NDIS supports are required), to justify the need for additional supports at a NDIS plan reassessment meeting, to support a Change in Situation request, or to simply gain a comprehensive overview of the individual’s functioning.
Cost: FCAs take typically 8 – 14 hours depending on age and complexity, totalling $1,551.92 – $2,715.86. Typically, paediatric clients will take 8 hours and adults will take 12 hours, with additional time added for complexity and/or if travel is required.
Individualised Living Options (FCA+ILO)
Typically for NDIS participants 18 and above. ILOs refer to a type of flexible support arrangement that may be funded by the NDIS. ILO assessment reports include a comprehensive FCA as well as additional information about support arrangements.
Supported Independent Living (FCA+SIL)
Typically for NDIS participants 18 and above. SILs refer to a 24/7 support arrangements for NDIS participants with higher support needs. SIL assessment reports include a comprehensive FCA as well as additional information and justification for SIL funding.
Cost: 2-3 hours is added onto the FCA assessment for the additional ILO or SIL component, depending on complexity. This takes the entire assessment/report to 14 – 17 hours, totalling $2,715.86 – $3,297.83.
SUBSIDISED NDIS ACCESS ASSESSMENTS
Suitable for all ages. We are proud to offer subsidised NDIS Access Assessment reports, where the client will pay for 2 x hours of information gathering (appointments and/or assessment scoring) and the report will be written free of charge. This will be similar to an FCA, but is structured more explicitly around the NDIS functional domains and helps in justifying the individual’s disability as meeting the NDIS eligibility criteria.
Cost: 2 hours = $387.98 (Medicare rebate available – check here for more info).
