Requests for access can be made through a Kalos Therapy manager. For security reasons, we may require identification before releasing the information.

- Access would interfere with an investigation or pose a threat to the safety of any individual
- Denial is required by law
We will handle all access and correction requests promptly. No fees are charged for accessing your information, though administrative fees may apply for copies.
